We are asking you to help us produce a record of historical buildings on or near Sparrow Lake (houses, cottages, hotels, barns, boat houses, etc.). We plan to begin with buildings that are 75 years old or older, because they are most at risk of demolition or other loss.

If you own, or know someone who owns, such a building we would very much appreciate it if you (or they) would fill in as much as possible the information requested below. (Please use one sheet for each property being described; you may photocopy this form if more are needed).

Be assured that this project is being undertaken for historical purposes only and the resulting files will have no bearing on your future use or alteration of the building, nor will the files be given or sold to any other party.
